The South Tryon Wellness Program focuses on the African-American community, in particular, because they statistically are at the highest risk for cardiovascular diseases. The purpose of the Wellness Program is to provide risk assessments, education and awareness of heart disease and its prevention, and free doctor visits for managing their Hypertension, Diabetes, and Heart Disease. In addition, the program encourages individuals to develop their own personal wellness plan to live a healthy heart lifestyle for themselves and their families. This is accomplished through one-one-one and fmaily diet consultations, educational classes, prescription and food assistance, and smoking cessation.
Although there is an abundance of free information about heart healthy living available to all people in this region, there is a void in getting this information and understanding of heart disease and its prevention to underserved African-Americans who are at the greatest risk for cardiac diseases. Therefore, the center's efforts are currently concentrated on the 12,000-citizen communities of Wilmore Park, Brookhill and Southside Park neighborhoods to heighten this awareness.
As this Wellness Program grows in awareness and need, our goal is to develop "floating" models throughout the Charlotte region to underserved neighborhoods. The outreach will evolve by strategic partnerships with organizations like South Tryon Community Development Corporation, Bethlehem Center, Neighborhood associations, and churches along with private corporations who will share the physical space with scheduled programming.
